Below you will find
the forms that need to be submitted to hold a place in a camp week. Forms (Registration, Health, Medication and Information: see below) with camp fee must be received by Camp Frederick 14 days prior to camper's arrival date. Be sure to attach a copy of your health insurance card and your child's immunization record to the Camper Health Form.
Please list a second choice week on the Camper Registration Form in the event that your first choice is filled or cancelled. You may call the office at 330-227-3633 to check on availability prior to submitting your forms.
Forms may be sent to Camp Frederick, PO Box 258, Rogers OH 44455, faxed to 330-227-9005 or emailed to info@campfrederickohio.com.
